We have an opening for a Community Health Worker (CHW). This position is responsible for helping patients and their families to navigate and access health and community services, other resources (such as addiction treatment, transportation, and food), and adopt healthy behaviors. The CHW will provide social support and informal counseling, advocate for individuals and community health needs, and provide health screening services.
We are looking for someone with a high school diploma or GED. Experience working in a multi-cultural setting and experience working in a community setting for at least 1-2 years preferred. Knowledge of some medical terminology preferred. Bilingual in English and Spanish is a big plus.

We are seeking an experienced Licensed Clinical Social Worker who is licensed in the State of California and has supervision experience. As the LCSW/Manager, you will spend half of your time with patients and the other half of your time managing the behavioral health therapists. You will provide screenings, consultation, behavioral interventions and provide short term counseling to patients within our integrated medical/behavioral health setting. You will also provide administrative oversight and clinical supervision to our behavioral health team.
