MCC’s Patient Advocates provide a unique service to our community. They are highly trained professionals who help low income or uninsured patients determine what state, federal, or alternative programs are available, such as: Medi-Cal; CalFresh; Education and enrollment assistance for Covered California health insurance; Sliding Scale discounts for low-income people; Medicare Part D selection assistance; Patient Assistance Program for medications.

What is the Outreach Department and what do they do?

The MCC Outreach Department coordinates with medical, dental and behavioral staff at MCC in simplifying access to State and Federal programs.

What State and Federal programs do MCC Outreach Department help with?

We assist with Medi-Cal and CalFresh applications, and screen patients for income-based sliding scale discounts which can reduce patient out-of-pocket expenses at the facility if they are uninsured. We assist with CoveredCalifornia applications, can help apply for Medicare and Social Security benefits or explain correspondence from the government about these things. The Outreach department also assists with permanent federal disability applications for SSDI (Social Security Disability Insurance) or SSI (Supplemental Security Income). Once a year, we assist with Medicare Part D screenings during the annual enrollment for medication benefits associated with Medicare.

Does the Outreach staff offer these services in other languages?

All three members of the Outreach department can assist patients in Spanish as well as English.

Are there any fees for this help?

No, the assistance is offered as a community service, free of any charge.

Do I have to be a patient at MCC?

No, this free, confidential service is available to anyone.
