MCC has the only medication assisted treatment program for opioid use disorders on the Mendocino coast. We are looking for a full time Case Manager to work with people in recovery from opioid use dependency. This position acts as a liaison with Suboxone Providers, integrated Behavioral Health Providers and other community agencies as well as connecting clients with other resources. The Case Manager meets with patients for intakes and assists in facilitating education, relapse prevention support group, and family outreach.
We are looking for someone with a high school diploma or GED. Experience working with substance use disorder and/or case management preferred. Knowledge of SUD services and recovery support services preferred. Bilingual in English and Spanish is a big plus.
We also have an opening for a Community Health Worker. This position is responsible for helping patients and their families to navigate and access health and community services, other resources (such as addiction treatment, transportation, and food), and adopt healthy behaviors. The Community Health Worker will provide social support and informal counseling, advocate for individuals and community health needs, and provide health screening services.
Qualifications include a high school diploma or GED; experience working in a multi-cultural setting and experience working in a community setting for at least 1-2 years preferred. Knowledge of some medical terminology preferred. Bilingual in English and Spanish is ideal.
